Technical Assistance:
Discussed children's proof of identification within 7 days of enrollment and new requirements for out-of-state background checks.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 10/14/2020 and concluded on 10/16/2020. The director was contacted by telephone to initiate the inspection. There were 97 children present and 22 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1) The record for Child #2 (dob 06/19/2019) did not contain a current immunization record. The most recent immunization report was dated 12/19/2019.

Plan of Correction: We will ask parents for an updated immunization record report.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain work phone number for both custodial parents. The record for Child #5 did not contain work phone number for one custodial parent.

Plan of Correction: We will have parents provide the information.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review, it was determined that the center did not ensure that employees who began employment after 07/01/2020 and that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Sex Offender Registry search request.
Evidence:
1) The record for Staff #3 (doh 07/20/2020) did not contain documentation of an out-of-state Sex Offender Registry search request. Staff #3 had declared in their sworn statement having lived out of the state of VA within the last 5 years before employment.

Plan of Correction: We will send out the out-of-state Sex Offender Registry search request immediately.
