Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 10/14/2020 and concluded on 10/16/2020. The director was contacted by telephone to initiate the inspection. There were 100 children present and 28 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain a work phone number for one custodial parent. The record for Child #5 did not contain a place of employment for one custodial parent.

Plan of Correction: We will ask the parents to provide the information.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, it was determined that the center did not ensure that Central Registry search results are obtained every five years for employees.
Evidence:
1) The most recent Central Registry search request results available in the record for Staff #2 were dated 07/05/2013. A new Central Registry search request was due on 07/06/2018.

Plan of Correction: A new Central Registry Release of Information Form was submitted on 10/21/2020.
