1. Procedures for sanitizing -
"Sanitized" means treated in such a way as to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution. The surface of the item is sprayed or dipped in the sanitizing solution and then allowed to air dry.
2. COVID-19 Phase III Guidance Update:
The Virginia Department of Labor and Industry (DOLI) approved an Emergency Temporary Standard that requires all staff in child care settings to wear cloth face coverings.

While masks are not required for children in child care, the essential guidance issued by the Department of Social Services on 9/25/2020, based on CDC recommendations, strongly recommends that children 2 years of age and older wear a face covering. This is a best practice that will decrease the risk of COVID-19 transmission when children and caregivers are in close proximity.

Programs that remain open are to adapt service settings to align with public health recommendations, which include social distancing and wearing face coverings. Review all information from Virginia Department of Social Services (VDSS), Virginia Department of Health (VDH) and the Centers for Disease Control and Prevention (CDC) regarding COVID-19 updates.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was conducted on 9/15/2020 with an exit interview on 10/23/2020. The director was contacted by telephone and a virtual inspection was conducted. There were 34 children present, ages three years - ten years of age, with eight staff supervising. The Inspector reviewed four children's records, four staff records, the emergency drill log, medication, meals and snacks, hand washing, enhanced cleaning and sanitizing of toys and surfaces, and daily indoor and outdoor activities. The information gathered during the inspection determined non-compliance with standards that are documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of four staff records, the center did not have documentation of a tuberculosis (TB) test/ screening for staff within 21 days of hire or within 12 months prior to hire. Evidence: the records for staff #1, #3 and #4 did not have documentation of a clearance statement signed by a physician, the physician's designee or an official of the local health department.

Plan of Correction: Will obtain TB tests/screening with proper documentation going forward.

Standard #: 22VAC40-185-160-B
Description: Based on review of four staff records, the center did not have an acceptable form of documentation of a tuberculosis (TB) test/ screening for staff. Evidence: the record for staff #2, did not have documentation of a negative TB skin test signed by a physician, the physician's designee or an official of the local health department.

Plan of Correction: Will obtain TB tests/screening with proper documentation going forward.

Standard #: 22VAC40-185-70-A
Description: Based on review of four staff records, it was determined that not all records contained all required information. Evidence: 1) The record for staff #1 did not have the address for an emergency contact or required documentation of two reference checks to include name, date and signature of person making the call. 2) The records for staff #2, #3, and #4 did not contain verification of age requirement for position, or an address for an emergency contact. 3) The records for staff #2 and #3 did not contain written information to demonstrate that the individual possesses the education or certification for the position.

Plan of Correction: Will obtain missing information

Standard #: 22VAC40-185-240-D-1
Description: Based on review of documentation, and an interview with staff, it was determined that there was not always a staff member on site with current Medication Administration Training (MAT) certification upon agreement to administer medication to children. Evidence: 1) the facility is open from 6:30 am - 6:00 pm Monday - Friday. 2) Staff with current MAT certification work from 7:30 am - 5:00 pm each day. 3) There is no staff member with MAT training in the building Monday - Friday from 6:30-7:30 am or from 5:00 - 6:00pm. 4) There are two emergency medications documented on site.

Plan of Correction: Staff are taking MAT certification now.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of four staff records, it was determined that not all staff had Central Registry search results within 30 days of employment. Evidence: The record for staff #1 (date of hire: 12/15/2019) did not have documentation of a central registry search report.

Plan of Correction: Staff no longer works here. Going forward all staff will have Central Registry search results within 30 days of employment.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of four staff records, it was determined that not all staff had an Office of Background Investigations (OBI) fingerprinting-based criminal history record check prior to their first day of employment. Evidence: 1) The record for staff #1 (date of hire: 12/15/2019) had a determination letter dated: 12/23/2019. 2) The record for staff #2 (date of hire: 8/20/2020) had a determination letter dated: 9/1/2020. 3) staff #3 (date of hire: 8/14/2020) had a determination letter dated: 9/11/2020.

Plan of Correction: Going forward all staff will have OBI fingerprint results prior to their first day of employment.
