Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 10/14/2020 and concluded on 10/16/2020. The director was contacted by telephone to initiate the inspection. There were 89 children present and 14 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-130-A
Description: Based on review of four children's records it was determined that not all records contained documentation of immunizations before the first date of attendance. Evidence: The record for child A (start date: 10/03/2019) did not have documentation of an immunization record on file.

Description: Based on review of four children's records it was determined that not all records contained documentation of a physical within the first thirty days of enrollment. Evidence: The record for child A (start date: 10/03/2019) did not have documentation of a physical on file.

Standard #: 22VAC40-185-340-A
Description: Based on interviews, it was determined that the center did not ensure the care, protection, and guidance of children in care. Evidence: The center failed to adhere to the health and safety guidelines established due to the COVID-19 pandemic of ensuring that children were maintaining social distance. Evidence: Six children were observed in the two year old room sitting closely on the circle-time carpet playing with a variety of manipulative toys.

Description: Based on interview and observation of the five year old room it was determined that the required staff to child ratio was not always maintained. Evidence: There were 11 children being supervised by 1 staff member in the five year old room. This five year old room is designated for Kindergarten-age children however, Child C (d.o.b 10/07/2015) does not meet the eligibility of turning five before the September 30 deadline.

Description: Based on review of three medications it was determined that medication was not returned within 14 days after the expiration of the authorization form. Evidence: The emergency medication on site for child B was accompanied by an authorization form which expired on 08/23/2020. The medication was not sent home or authorization form renewed by 09/10/2020
