This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated and concluded on 10/22/2020. The center supervisor was contacted by telephone to initiate the inspection. There were 24 children present and 13 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-520-B-11 Description: Based on record review, the center did not ensure that a written allergy care plan, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction, was obtained for each child with a diagnosed food allergy.
Evidence:
1. Child #1 has a diagnosed food allergy. The allergy care plan in the record for Child #1 did not contain a physician and parent's signature.Plan of Correction: We will talk with the parent today about obtaining a plan with the doctor's signature.
