Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An "other" monitoring inspection was initiated on 09/30/2020 and concluded on 10/27/2020. A self-reported incident was received by the department regarding allegations in the area of programs. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation. The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Violations:
Standard #: 22VAC40-185-440-K
Description: Based on interviews, a record review and observations, the center did not ensure crib sides were up and fastenings were secured when a child was in the crib.
Evidence:

A. On 09/30/20 around or about 1:15pm, staff #1 unfastened the side of a crib, bringing its side down and placing 8 month-old baby #1 inside. Staff #1 reported bringing the crib side back up before leaving the crib to attend to another baby. The crib side was not fastened and the infant fell 1.5 feet and landed on the floor.

B . None of the 4 staff present witnessed baby #1 fall out of the crib, however, three reported hearing baby #1 cry, turning their attention to the baby lying on the floor with the crib side down and unfastened.

C. The director removed the crib from the classroom and tested the crib sides for a secure fastening. The director reported no evidence of defect in the fastening mechanism, confirming the crib was not fastenend before staff #1 stepped away from the crib.

Plan of Correction: CHG added crib operations to the current Infant room training from. All current infant staff will be trained on this update on or before 11/06/2020. This includes but is not limited to properly securing crib sides, but also as teachers roam to check on sleeping infants to also check (tug on the side of the) crib. All new staff will also be trained on these procedures. CD, AD or designated training staff will train all current and new infant room staff.
