A renewal inspection was initiated on October 27, 2020 and concluded on October 28, 2020. The provider was contacted by telephone to initiate the inspection. There were 83 children present and 15 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed five children?s records, four staff records, and three board members submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-130-B Description: Based on a review of children's records, the provider did not ensure each record contained additional immunizations once every six months for children under the age of two years.
Evidence:
The record for child #3 (date of birth 02/15/19), had one immunization record documented on 08/16/19.Plan of Correction: Director has requested the information
Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, the center did not ensure that each record contains all required information.
Evidence:
1. The record for child #1 did not contain the address for two designated people to call in an emergency.

Standard #: 22VAC40-185-510-N Description: Based on a record review, the center did not have written authorization from the child's physician and parent for long-term prescription drug use.
Evidence:
The record for child #6 contained out of date authorizations for long-term medications, dated 02/04/20.Plan of Correction: Director has requested updated medication authorizations.
