Technical Assistance:
Training given regarding additional requirements in subsidy. Discussion about lockdown procedures discussed.

Comments:
An unannounced SHSI inspection was conducted today. There were 5 children present with the vendor (1 was the Vendor's child) for a total of 11 points. The Vendor had paperwork for the enrolled children and had a variety of supplies available for the children. The children were observed playing and sharing the toys. The Inspector discussed and trained the vendor on record keeping for drills and attendance. Please call with questions 703-479-4679 cynthia.ford@dss.virginia.gov

Standard #: 22VAC40-665-160-B
Description: Based on record review it was determined the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of one child in care as it occurs. Evidence; 1. The vendor had notebook of individual attendance logs for each child enrolled. Child 4 is only part time and was present today. The vendor stated the child only comes one or two days a week. 2. The attendance log had child 4's last attendance in July but vendor said she forgot to keep up with the attendance.

Plan of Correction: Inspector gave me a sheet where I could put all children down on one sheet.

Description: Based on record review and interview, it was determined the vendor did not conduct the shelter-in-place procedures in the last year. Evidence: 1. Vendor had a sheet with monthly evacuation drills but said she didn't conduct shelter-in -place.

Plan of Correction: Inspector gave me a sheet and I will use. I will have one this month.

Description: Based on record review and interview, it was determined the vendor did not conduct the lockdown procedure in the last year. Evidence: 1. The vendor said she didn't practice it.

Plan of Correction: I will conduct it this month and use the sheet the inspector gave me.
