Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 11/10/20 and concluded on 11/10/20. The resource specialist was contacted by telephone to initiate the inspection. There were 47 children present and 9 staff. The inspector emailed the resource specialist a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, 4 of 4 children's records did not contain all required information.

Evidence:
1. The record for Child #1 did not contain the name, address, and phone number for one of two designated people to call in an emergency if a parent cannot be reached.
2. The records for Child #2, 3, and 4 did not have documentation of the date of viewing proof of the child's identity and age.

Plan of Correction: We will ask the parent today for a second emergency contact.
Moving forward, we will put a place on our form to record the date proof of id is viewed.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, the center did not obtain a fingerprint-based background check for each staff member prior to their first day of employment.

Evidence:
1. Staff #1 began employment on 2/3/2020. The fingerprint-based background check in the record for Staff #1 was dated 2/5/2020.
2. There was no documentation of a fingerprint-based background check in the records for Staff #2 and Staff #3.

Plan of Correction: The center will provide documentation that the requests are sent within 10 business days from the date of this inspection.
