Reviewed new background requirements, effective 7/1/2020, for individuals who reside outside of the state of Virginia or who have resided outside of the state of Virginia in the past 5 years as well as timelines in which each background requirement must be completed.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 11/13/2020 and concluded on 11/13/2020. The director was contacted by telephone to initiate the inspection. There were 11 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 2 staff records, and 2 agent records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-130-A Description: Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1.Child #1 began attending the child day center on 09/08/2020. Immunizations in the record for Child #1 were dated 9/18/20.
2. Child #2 began attending the child day center on 09/07/2019. Immunizations in the record for Child #2 were dated 10/17/2019.Plan of Correction: Moving forward, families will be contacted in July, prior to the beginning of the school year if immunizations have not been received and notified that children cannot start until they are received.
Standard #: 22VAC40-191-40-D-1-A Description: Based on record review, the records for 2 of 2 agents of the child day center did not contain a sworn statement or affirmation, search of central registry and criminal history record check.
Evidence:
Documentation of a sworn statement, central registry check, and fingerprint-based criminal history record check for Agent #1 and Agent #2 was not available for review during the inspection.Plan of Correction: The center will provide documentation that the backgrounds have been completed or that the requests were sent within 10 business days.
