Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated and concluded on November 5, 2020. The program director was contacted by telephone to initiate the virtual, remote inspection. There were 53 children present with nine staff. The inspector reviewed a list of items required to complete the inspection. The inspector reviewed three children?s and staff records during the virtual, remote inspection to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-350-C
Description: Based on observation and interview the center failed to ensure that when children are in ongoing mixed age groups the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.
Evidence:
1 - During naptime, in the two year old room there are 11 children resting and sleeping with one staff, staff 2. The youngest child in the group, child 1, is 23 months old. Therefore, an additional staff is required for the group to be ratio.
a. The record for child 1 lists a date of birth as November 23, 2018.
2 - Staff 1 confirmed that there were 11 children in present in the two year old room and the youngest child present was 23 months old.

Standard #: 22VAC40-185-350-F
Description: Based on observation, interview and record review the center failed to ensure that with parent's written permission and a written assessment by the program director and program leader, a center may choose to assign a child to a different age group if such age group is more appropriate for the child's developmental level and the staff-to-children ratio shall be for the established age group.
Evidence:
1 - Child 1 was present in the two year old room, during naptime prior to the child's second birthday without the required documentation from parents, program director or program leader on file.
2 - Staff 1 stated that the center had not collected the required parent signature nor had staff completed the required developmental scale for child 1 to be present in the two year old classroom with a 1:8 ratio.

Description: Based on observation and interview the center failed to ensure that there is a one day's emergency supply of formula available for formula fed infants.
Evidence:
1 - The expiration date on the center maintained emergency infant formula was October 30, 2020
2 - Staff 1 stated that the emergency formal had expired on October 30, 2020.
