Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on November 17, 2020 and concluded on November 17, 2020. The director was contacted by telephone to initiate the inspection. There were 86 children present and 17 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed five children?s records and five staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:

Standard #:	22VAC40-185-130-A
Description:	Based on review of children's records, the facility did not obtain documentation of immunization as required by the State Board of Health prior to the child's first day of attendance. Evidence: The record of child #3, enrolled 09/04/2018, had documentation of immunizations dated 09/10/2018, exceeding the first day of attendance.
Plan of Correction:	Per the director: We will ensure administration and staff are aware that documentation of immunizations must be received by the center before a child can attend.
Standard #:	22VAC40-191-40-D-1-A
Description:	Based on review of documentation and interview, the facility did not obtain a sworn statement or affirmation and a central registry search within 30 days of the center designating an agent or board officer. Evidence: 1. Individual #1, identified as a agent on the renewal application, did not have documentation of a completed central registry form. The director stated the agents central registry could not be located. 2. Individual #1, identified as a agent on the renewal application, did not have documentation of a completed sworn statement. The director stated the agents sworn statement could not be located.
Plan of Correction:	Per the director: The agents original central registry and sworn statement could not be located. Going forward we have requested a new central registry and the agent completed an updated sworn statement. We will maintain the background checks for the agent on site for future reference.
