Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection during the conditional licensure period for this site was initiated and concluded on 11/16/2020. The area manager was contacted by telephone to initiate the inspection. There were 12 children present and 3 staff. The inspector emailed the area manager a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete..
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of records and interview, the licensee did not ensure that each enrolled child's record contained the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Evidence:
Three children's records were reviewed. The record for child #1 contained only one of the two required emergency contacts.

Plan of Correction: The licensee will obtain the name address and phone number of a second emergency contact by the end of the week and place this information in child #1's record.
