Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on November 19, 2020 and concluded on November 19, 2020. The director was contacted by telephone to initiate the inspection. There were 79 children present and 15 direct staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed four children?s records and four staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-40-G
Description: Based on record review, the center did not obtain proof of a child's identity and age requirement as stated in 63.2-1809 of the Code of Virginia.

Evidence:
1. Child #3's record did not contain documentation of proof of identity. Child #3's date of enrollment was 10/05/2020.Child #3 transferred from another center that was located in Washington DC.

Plan of Correction: The center has contacted the parent regarding the proof of birth. The parent will obtain proof of birth.

Standard #: 22VAC40-185-70-A
Description: Based on record review, the center did not ensure that two references were checked prior to employment.

Evidence:
1. Staff #1's record(Date of Hire-10/26/2020) had documentation of two references dated 10/30/2020.

Plan of Correction: Moving forward, the center will ensure that all references are checked prior to employment.

Standard #: 22VAC40-185-420-E-3
Description: Based on record review, the center did not request annual parent confirmation that the required information in the child's record was up to date.

Evidence:
1. Child # 1's and Child #4's record did not contain documentation of an annual review. Child #1's first day of attendance was 01/03/2017. Child #4's first day of attendance was 10/31/2016.

Plan of Correction: The center will create a yearly form for parents to complete and date to ensure information is current and correct. The form will be placed in the record.

Standard #: 22VAC40-191-60-B
Description: Based on record review, two of four staff records did not include documentation of a completed Sworn Disclosure Statement(SDS) prior to the first day of employment.

Evidence:
1. Staff #1's (Date of Hire-10/26/2020) record contained documentation of a completed SDS dated 10/27/2020.
2. Staff #2's (Date of Hire-10/26/2020) record contained documentation of a completed SDS dated 10/28/2020.

Plan of Correction: Moving forward, the center will ensure that all Sworn Disclosure Statements are completed prior to employment or the first day of employment.
