Standard #: 22VAC40-665-160-B Description: Based on the review of the vendors paperwork, the vendor failed to keep an up to date attendance record. Evidence: The vendor did not keep attendance this month and also failed to record times of arrival and departure. Plan of Correction: I will make sure I keep attendance the correct way , it was an issue getting parents to sign so I glad I can keep attendance without that.
