Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 11/17/20 and concluded on 11/17/20. The director was contacted by telephone to initiate the inspection. There were 17 children present and 2 staff present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of three children's records, it was determined that the facility did not ensure that the center obtains documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
1. There was not an immunization record available for viewing during the inspection for child #1.
2. There was not an immunization record available for viewing during the inspection for child #2.
3. Staff #4 confirmed there was no immunization record available for viewing during the inspection for child #1 and child #2.

Plan of Correction: The facility responded: We were unable to access a copy of the immunization record electronically at the site during the inspection due to a technical issue. There is a copy of the current immunization record and our central office and it was sent to the Licensing Inspector. We are working with our IT Department to solve the technical issue.. All new program participants will be required to have a current immunization record prior to entering the program.
