This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 11/19/2020 and concluded on 12/1/2020. The Director was contacted by telephone to initiate the inspection. There were 64 children present with 11 staff. The inspector emailed the Director a list of items required to complete the inspection. The inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.
The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility.
The licensing inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.
Standard #: 22VAC40-185-130-B Description: Based on record review and interview, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
1. Child #1 (date of birth: 11/14/2019) most recent documentation of immunizations is dated 1/10/2020.
2. Child #4 (date of birth: 7/11/2019) most recent documentation of immunizations is dated 11/13/2019.
Plan of Correction: I will send an email to the parents today to request the immunizations.
Standard #: 22VAC40-185-60-A Description: Based on record review and interview, children's records do not contain all required information.
Evidence: Child #4 record does not contain a second emergency contact.

Standard #: 22VAC40-665-580-K Description: Based on interview, there is not always at least one staff on duty who has obtained within the last three years instruction in performing a daily health observation of children.
Evidence: The program director confirmed that staff #5 is the only staff that has current daily health observation training. The program director confirmed that staff #5 is not on-site during all of the schools hours of operation.
Plan of Correction: We were waiting to coordinate a training for additional staff to become trained. We'll use the VDSS online training to get an additional morning and an additional afternoon staff person trained.
