Technical Assistance:
1. Reviewed/trained vendor on new subsidy requirements. 2. Forms provided: attendance, and new emergency plan.

Comments:
1:20-2:40pm Annual unannounced subsidy inspection completed. Observed 1 child + 3 children that live in the home = 7 points with the vendor. Ratios and supervision were in compliance. The child in care was being held, playing with toys, and lying in crib. The home was found to be very clean, safeguarded appropriately, and well supplied with toys and equipment for the children. The vendor completed the annual child development training requirements, and is currently certified in First Aid, CPR, and MAT. Recordkeeping non-compliance found.

Description: Based on records reviewed, it was determined that 4 of 4 children's records did not include required information. Evidence: 1. Child #5 - Home/work address for 1 of 2 parents; and address for the emergency contact person. 2. Child #6 - Home address for 1 of 2 parents; and address for the emergency contact person. 3. Child #7 - Home address for 1 of 2 parents; and address for the emergency contact person. 4. Child #8 - Address for the emergency contact person.

Plan of Correction: The vendor will ask parents to complete the required information.
