Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 11/24/20 and concluded on 12/3/20. The director was contacted by telephone to initiate the inspection. There were 62 children present with 16 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records, 4 staff records and 1 child?s medication and supporting documents submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and interview, the center failed to obtain all required information for one out of four children's records.

Evidence:

1. The record for child 4 contained information for the mother and a step-mother.
2. The director stated the child's record did not contain any information regarding the father.

Plan of Correction: The information regarding child 4's father will be obtained and added to the child's record. In the future children's records will be reviewed received to ensure all required information is provided.

Standard #: 22VAC40-185-550-D
Description: Based on record review and interview, the center failed to conduct monthly practice evacuation drills.

Evidence:

1. The documentation of the monthly evacuation drills did not have drills documented as being conducted for 11/20.
2. The director stated a drill was scheduled for the last day of November but due to bad weather conditions they were unable to conduct the drill and did the drill on the first of December instead.

Plan of Correction: An evacuation drill was conducted 12/1/20. A drill was planned for 11/30/20 but it rained. In the future an evacuation drill will be conducted every month.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review and interview the center failed to ensure a staff hired after 7/1/20 that lived outside VA within the last five years out of state sex offender search on file before they begin employment.

Evidence:

1. The record for staff 4, start date 11/6/20, documents living outside of VA within the last five years. There was no out of state sex offender search on file.
2. The director stated they did not conduct an out of state sex offender search.

Plan of Correction: An out of state sex offender search will be completed for staff 4. In the future, no staff will begin employment that has lived outside VA in the last five years until an out of state sex offender search is completed.
