Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 12/8/2020 and concluded on 12/11/2020. The Director was contacted by telephone to initiate the inspection. There were 32 children present with 7 staff. The inspector emailed the Director a list of items required to complete the inspection. The inspector reviewed 3 children?s records and 5 staff records submitted by the facility to ensure documentation was complete.

The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-140-B
Description: Based on record review and interview, the physical exam of a child over the age of two was more than 12 months old.
Evidence: Child #3 (Date of enrollment: 8/6/2020) is 7 years old. The most recent physical exam for Child #3 is dated 11/10/15.

Plan of Correction: I'll ask the mom tonight for an updated copy of the physical.

Standard #: 22VAC40-185-160-C
Description: Based on record review and interview, staff did not obtain and submit the results of a subsequent tuberculosis (TB) screening every two years.
Evidence: Staff #2's most recent TB screening is dated 11/12/2017.

Plan of Correction: I've been asking them to get it done. They'll get it done next week.
