Technical Assistance:

1. Reviewed COVID-19 health + safety guidance, and discussed the additional safety precautions taken by the center. 2. Guidance provided on background check requirements/laws. Current guidance, laws, and links for out-of-state checks is on the VDSS website. 3. Staff are required to have background checks within the required timelines in order to work at the center.

Comments:

This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An on-site inspection of the center was not conducted at this time, and efforts to complete a virtual visual inspection were unsuccessful due to poor cell service at the center. A unannounced monitoring inspection was conducted on this date. The director was contacted by phone to initiate the inspection. There were 16 children + 5 staff were present today. Ratios + supervision were in compliance. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 2 child records + 2 staff records to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or laws, and violations were documented on the violation notice. Violations:

Standard #:	22VAC40-185-160-A
Description:	Based on records reviewed, it was determined that 1 of 2 staff records did not include a negative TB screening within 21-days of employment. Evidence: Staff #5 - Date of hire was October 2019, and to-date their is not TB screening on-file.
Plan of Correction:	The director said the TB test is in process. Moving forward, TB tests will be required and completed within the first 21-days of employment.
Standard #:	22VAC40-191-60-C-2
Description:	Based on records reviewed, it was determined that 1 of 2 staff records did not include the results of a central registry check (CPS) within 30-days of employment. Evidence: Staff #5 - Date of hire was October 2019, and the CPS check was completed on 12/19/20.
Plan of Correction:	The CPS check will be submitted within 10-days, and the results will be sent to the inspector for review. Moving forward, CPS will be done within first 30-days of employment.
Standard #:	63.2(17)-1720.1-B-3
Description:	Based on records reviewed, it was determined that 2 of 2 staff records did not include a completed out-of-state central registry check (CPS) by the original deadline in 2018 for existing employees; and within first 30-days of employment for those hired after that date. Evidence: Staff #1 - Date of hire was June 2016, the out-of-state CPS check was due to be completed in 2018, and to-date no out-of-state CPS check has been completed.
Plan of Correction:	The CPS check will be submitted within 10-days, and the results will be sent to the inspector for review. Moving forward, out-of-state CPS will be done within first 30-days of employment.
