Provided consultation on background checks:

All existing employees who have lived in another state within the past five years must complete a state criminal history name check and sex offender registry check from the other state. Results must be received before December 31, 2020.

All employees (who have lived in another state within the past five years) hired after July 1, 2020 must have this criminal history name check and sex offender registry check prior to employment.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A focused monitoring inspection was conducted on 12/20/2020 with an exit interview on 12/14/2020. The director was contacted by telephone and a virtual inspection was conducted. There were 41 children present with 13 staff supervising. The Inspector reviewed three children's records and seven staff records as well as enhanced hand washing and cleaning and sanitizing of toys, surfaces, and playground equipment. The information gathered during the inspection determined non-compliance with standards that are documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on review of seven staff records, the facility did not ensure that each staff record contained all required information. Evidence: 1) the record for staff #7 did not have verification of age as required for the position. 2) the records for staff #1, #2 and #3 did not have the name, address and telephone number of a person to be notified in an emergency. 3) the records for staff #1, #2, #3, #4, #5 and #7 did not have written information to demonstrate that the individual possesses the education, staff development, certification, and experience required by the job position.

Plan of Correction: Going forward all files will be complete before a staff member starts.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of seven staff records, it was determined that no Out of State Child Abuse and Neglect Search was requested by the end of the 30th day of employment. Evidence: the record for staff #5, (date of hire: 10/26/18) who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of state search request.

Plan of Correction: Going forward all background checks will be completed in the required time frames.
