This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A Monitoring inspection was initiated on 12/4/2020 and concluded on 12/14/2020. The Director was contacted by telephone to initiate the inspection at 10:50am. There were 37 children present and 10 staff. The inspector emailed the Director a list of items required to complete the inspection. The Inspector reviewed 3 children's records and 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-70-A Description: Based on a review of staff records, 1 out of 4 staff did not have documentation of 2 or more references on file.
Evidence:
1. Staff #1 had documentation of one out of two required references on file.Plan of Correction: A second reference was received on the same day as the inspection.
Standard #: 22VAC40-185-550-D Description: Based on a review of records and staff interview, the center did not practice an evacuation drill monthly.
Evidence:
1. There was no documentation of an evacuation drill for November.
2. The Director stated that there was not an evacuation drill conducted in November.Plan of Correction: We will do evacuation drills monthly.
Standard #: 22VAC40-191-60-B Description: Based on a review of staff records, 2 out of 4 staff did not complete a sworn statement or affirmation before employment.
Evidence:
1. Staff #1 (doh: 6/17/2020) had a sworn statement on file that was dated 9/25/2020.
2. Staff #2 (doh: 8/24/2020) had a sworn statement on file that was dated 8/26/2020.Plan of Correction: We will make sure staff sign the sworn statement or affirmation on or prior to their first date of employment.
