63.2(17) License & Registration Procedures

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A (virtual) renewal inspection was initiated on 12/15/2020 and concluded on 12/16/2020. The director was contacted by telephone to initiate the inspection. There were 41 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-191-60-C-2 Description: Based on a review of staff records, the center did not have a central registry finding within 30 days of employment or volunteer service.
Evidence: Staff B's (date of hire: 10/12/2020) record did not contain documentation of central registry findings.Plan of Correction: I will make sure we have all background request results before employees are hired and document follow-up appropriately.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on a review of staff records, the center did not obtain a copy of information from the central registry.
Evidence: Staff A's (date of hire: 10/12/2020) record indicated that the employee has lived in Maryland within the past five years. Staff A's record did not contain documentation of Maryland central registry results.Plan of Correction: I will make sure we have all background request results before employees are hired and document follow-up appropriately.
Standard #: 63.2(17)-1720.1-B-4 Description: Based on a review of staff records, the child day center did not obtain a copy of the results of a criminal history record information check and sex offender registry check from any state in which the applicant has resided in the preceding five years.
Evidence: Staff B's (date of hire: 10/12/2020) record indicated that the employee has lived in the District of Columbia within the past five years. Staff B's record did not contain documentation of criminal history and sex offender search results.Plan of Correction: I will make sure we have all background request results before employees are hired and document follow-up appropriately.
