Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was initiated on 12/10/20 and concluded on 12/16/20. A complaint was received by the department regarding allegations in the areas of Physical Plant. The director was contacted by telephone to conduct the investigation. The evidence gathered during the investigation supports the allegation of non-compliance with standards or law and violations were issued.

Description: Based on interviews, a record review and observations, the center did not ensure all areas of the center were maintained in a clean and safe manner.
Evidence:
A. In the pantry evidence of rodents was observed.
1. Feces droppings were observed on packages of food designated for the children.
2. Several food packages were torn and food had spilled from some of them.
3. Paper products stored in the kitchen were torn through.
3. Staff reported seeing 4 dead mice in the kitchen area.

Plan of Correction: The center has temporarily closed and is working on a list provided by the Health Department to alleviate issues stated above.
