This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 12/08/2020 and concluded on 12/08/2020. The director was contacted by telephone to initiate the inspection and by video call to conduct the inspection virtually. There were 49 children present and 10 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.
The provider's responses for the 'plan of correction' were not received as of 12/21/2020 and will not appear on this Violation Notice.
Standard #: 22VAC40-665-520-B-11 Description: Based on a review of records and interview on 12/08/2020, the vendor did not obtain a written allergy care plan for two out of five children with a diagnosed food allergy.
Evidence: 1) The most recent physical examination in the record for Child #2 indicated the child has a diagnosed food allergy, but the record did not contain a written allergy care plan. 2) The most recent physical examination in the record for Child #4 indicated the child has a diagnosed food allergy, but the record did not contain a written allergy care plan. 3) During interview, a member of management confirmed the center does not have a written allergy care plan for Child #2 or for Child #4. A child's record shall include a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Plan of Correction: Not available online. Contact Inspector for more information.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
