Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A self reported investigation inspection was initiated on 10/25/2020 and 11/20/2020 and concluded on 12/10/2020.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-410-1
Description: Based on written interviews with staff, the center failed to ensure that physical punishment, striking a child, roughly handling or shaking a child was not used as a form of punishment.

Evidence:
1. Staff # 2 stated that she witnessed staff # 1 spank Child # 1. Staff # 1 verified that she did spank the child and that it was a mistake and was sorry. There was no visible marks on child # 1.

Plan of Correction: Due to center's policies the staff person was let go. The administration had staff review all policies on Forbidden actions.
