This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on October 23, 2020 and concluded on December 15, 2020. A complaint was received by the department regarding allegations in the areas of supervision of children and environmental safety. The director was contacted by telephone and video call to conduct the investigation. The inspector discussed with the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and a violation was issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-185-270-A Complaint related: Yes Description: Based on interviews, observations and documentation review, the center did not maintain all areas and equipment in a clean, safe and operable condition.
Evidence:
During phone calls, a video call and emails the following information was determined:
1. Staff #1 and Staff #2 stated a little before lunchtime they saw Child A (three years old) and another child crawl underneath the child size computer desk that was located in the Blue Room on 10/16/2020.
2. Staff #1 stated she asked both children to come out from under the computer desk. Staff #1 stated [Child A] scooted out backwards, head first, while lying on her back and the computer monitor fell off the desk. Staff #1 stated the base of the computer monitor hit [Child A] on the forehead above the eye, creating a cut. Staff #1 stated she took [Child A] to the classroom bathroom while Staff #2 called the Get Well station. Staff #1 stated [Child A] was crying and bleeding while she cared for her in the bathroom.
3. Staff #2 stated she saw [Child A] sliding out from the computer desk while lying on her back and the computer monitor fell, hitting [Child A] on the head. Staff #2 stated she called Get Well station to get help.
4. Staff #3 stated she received a phone call from one of the Blue Room teachers on 10/16/2020 stating a child was hurt. Staff #3 stated she went to the Blue Room and saw Staff #1 applying pressure to the forehead of [Child A]. Staff #3 stated she took a ?quick peek at the cut and knew right away [Child A] would need stitches?. Staff #3 stated she called the parent of Child A and a parent picked up [Child A].
5. In an interview with Non-Staff #1, the individual stated [Child A] was taken to a local emergency room where [Child A] received eight stitches and medical glue for a cut over the left eye that was approximately two inches long, one inch wide and a ? inch deep.
6. An incident report completed by the center dated 10/16/2020 states ?[Child A] crawled under the computer desk and knocked the computer monitor down onto her forehead?. The description of the Incident/Injury indicated ?approx. 1.5-2 laceration on forehead?.
7. Medical records received 12/12/2020 indicated [Child A] was treated on 10/16/2020 for ?2 cm laceration to left side of forehead? that was closed with eight sutures and Dermabond.8. The licensing inspector received pictures of Child A showing sutures over the child's eye.
9. Staff #1 stated she did not know what caused the monitor to fall off the desk. Staff #2 stated she was not sure what caused the monitor to fall but ?for sure remembers cords draped?.Plan of Correction: Immediately following the incident, the computer equipment and desk were removed from the classroom for inspection to determine if any flaws were present. None were found. The computer desk is intended for child use and was being used for the purposes as designed to hold a desktop, monitor, and keyboard for use in an early education classroom setting. The teachers did not believe the computer monitor to be a hazard prior to the injury. The computer equipment, monitor, and desk were in good repair at the time of the injury. The child was under direct sight and sound supervision by teaching staff before, during and after the injury. The incident was reviewed and an immediate safety assessment was completed. The school staff (classroom and administration) will continue our established practice of routine safety checks to ensure all equipment is in a safe and operable condition. All staff are required to complete ongoing safety checks, and the School Administrative team is responsible for monthly quality assurance checkpoints.
