Discussed with center Director, Phase III recommendations and COVID-19 Essential Guidance for Child Care programs:
Groups of children may share the same physical space (e.g. classroom, gymnasium) as long as they maintain social distancing of six feet.
The same staff and children should consistently remain in the same groups to the greatest extent possible.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 12/9/2020 and concluded on 12/11/2020. The Education Specialist was contacted by telephone to initiate the inspection. There were 112 children present and 24 staff. The inspector emailed the Education Specialist a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review and interview, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
1.Child#2's record contains immunization records that were last updated on 8/28/2019.
2.Child#3's record contains immunization records that were last updated on 3/8/2019.
3.Child#5's record contains immunization records that were last updated on 5/23/2019.

Plan of Correction: Emailed parents to obtain immunizations on 12/16/2020. As of 12/18/2020 we have received child#3 and #5 immunization records.

Standard #: 22VAC40-185-350-E-2
Description: Based on interview and documentation reviewed, the center did not maintain the required staff-to-child ratio applicable to the youngest child in a group.

Evidence:
1.During the inspection, there were 12 children in care in the Toucans classroom, being supervised by 2 staff.
2. Center management confirmed that the youngest child in the class is 23 months old.
3. Children 16 months old to 2 years old require a ratio of one staff for every five children.

Plan of Correction: Maintain ratio. Had parent sign transition letter and teacher conduct assessment.

Standard #: 22VAC40-185-550-D
Description: Based on review of emergency drills, the center did not conduct two required evacuation drills.

Evidence:
There was no documentation of evacuation drills practiced for the months of July 2020 and September 2020.

Plan of Correction: Make sure all fire drills are documented and conducted monthly.
