Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on December 15, 2020 and concluded on December 21, 2020. The center was contacted by telephone to initiate the inspection at 10:30 a.m. A virtual inspection was conducted beginning at 11:00 a.m. There were 83 children in care with 13 staff. The inspector emailed the principal a list of items required to complete the inspection. The Inspector reviewed five children?s records and five staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #:	22VAC40-185-140-A
Description:	Based on review of documentation, it was determined that the center did not obtain documenation of a physical exam from a physician for all children within 30 days of enrollment. Evidence: Child #4, enrolled 8/24/2020, did not have a physical in the record submitted.
Plan of Correction:	The required forms will be kept up-to-date in the specified time.
Standard #:	22VAC40-185-260-B
Description:	Based on review of documentation, it was determined that the facility did not provide documentation of an annual health inspection for the facility. Evidence: During the remote, virtual inspection initiated on 12/15/2020, no documentation of a health inspection was provided as requested by 12/21/2020.
Plan of Correction:	The health department has been contacted for the documentation.
Standard #:	22VAC40-185-340-A
Description:	Based on observation, it was determined that the center did not ensure the care, protection and guidance of all children in care. Evidence: 1. The center failed to adhere to the health and safety guidelines established due to the COVID-19 pandemic of maintaining social distancing of at least six feet between individuals. 2. Children over the age of two years of age were not using cloth face coverings when physical distancing could not be maintained. 3. On December 15, 2020, in the Preschool classroom, 20 children were observed side by side on the carpet and in various areas of the classroom together. Social distancing was not implemented nor were children wearing cloth face coverings.
Plan of Correction:	Health and safety guidelines will be maintained and additional measures will be in place to adhere to social distancing requirements. For example, a part of the class may do circle time while others are doing individualized activities.
