Technical Assistance:
Out-of-state background check requirements reviewed. Written information emailed.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the governor of Virginia.

A monitoring inspection was conducted on 12/16/2020. The licensing manager was contacted by telephone to initiate the inspection. There were no children present as the facility has been closed since 03/16/2020. Virtual learning is being offered at this time. The inspector emailed the licensing manager a list of items required to complete the inspection. The inspector reviewed three children's records and three staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the Licensing Manager COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the center failed to ensure that staff records contain all of the required elements.

Evidence:
1. The record for staff 1 and staff 2 lacked documentation of orientation training required by the end of the individuals first day of employment.
a. Staff 1 has a hire date of 03/18/2019.
b. Staff 2 has a hire date of 06/03/2019.
2. Administrative staff confirmed that documentation of orientation training could not be located.

Plan of Correction: Documentation of orientating training will be located and added to employment files.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and interview, the center failed to obtain a search of the central registry from any state the employee has resided in within the past five years.

Evidence:
1. The results of a central registry check from the State of Connecuticut are not on file for staff 1.
a. Staff 1 has a hire date of 03/18/2019 and indicated on her sworn statement or affirmation she had resided in the State of Connecticut within the past five years.
2. Administrative staff confirmed that the results of a central registry check had not been obtained for staff 1.

Plan of Correction: A central registry check from the State of Connecticut was requested on 12/30/2020. The results will be forwarded to the inspection when received.

Description: Based on record review and interview, the center failed to obtain a copy of the results of a criminal history record information check from any state in which the individual has resided in the preceding five years.

Evidence:
1. The results of a criminal record check from the State of Connecticut are not on file for staff 1.
a. Staff 1 has a hire date of 03/18/2019 and indicated on her sworn statement or affirmation she had resided in the State of Connecticut within the past five years.
2. Administrative staff confirmed that the results of a criminal record check had not been obtained for staff 1.

Plan of Correction: A criminal record check from the State of Connecticut will be requested as soon as possible.
