This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on Tuesday, January 5, 2021 and concluded on Friday, January 8, 2021. The director was contacted by telephone to initiate the inspection. There were 50 children present and 10 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.

Standard #: 22VAC40-185-140-A Description: Based on review of children record and interview with staff, the center did not ensure to obtain physical within one month after attendance for all children.
Evidence: In Child#2's record (start date: 07/06/2020), the physical documentation provided was incomplete. Interview with staff confirmed the documentation provided for review was what was available during this inspection.Plan of Correction: The center will follow up with the parent to obtain complete physical documentation and include in the child's record.
