Technical Assistance:
The Licensing Inspector has reviewed with the director COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated and concluded on 01/08/2021. The director was contacted by telephone to initiate the inspection. There were (101) children with (18) classroom staff, in (11) groups. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed (5) children?s records and (5) staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review, 3 of 5 children files indicated that not all of the required information/documentation was contained within each child's record.

Evidence -
1. On the date of inspection, 01/08/2021, health informationas required by 22 VAC40-185-130 (immunizations) through 22 VAC40-185-150 (physical) was not available for review for: Child #1 (start date: 07/01/2019), and Child #4 (start date: 10/05/2020).
2. Documentation of viewing proof of the child's identity and age was not available for the following children: Child #3 (start date: 01/07/2019), and Child #4 (start date: 10/05/2020).

Plan of Correction: All missing documentation shall be obtained and placed in the applicable child's file.

Standard #: 63.2(17)-1720.1-A
Description: Based on review, 1 of 5 staff files indicated that the center did not obtain a copy of a search of the central registry (CPS check) from any state in which the staff person has resided in the preceding five years to determine if the staff person was the subject of a founded complaint of child abuse or neglect outside the Commonwealth.

Evidence -
1. On the date of inspection, 01/08/2021, a CPS check from the State of West Virginia for Staff #5 was not available for review.
2. The start date for Staff #5 was listed as 09/08/2014.

Plan of Correction: The missing documentation shall be obtained and placed in the staff person's file.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on review, 2 of 5 staff files indicated that the center did not obtain a copy of the results of a sex offender registry (SOR) check from any state in which the staff person has resided in the preceding five years.

Evidence -

1, On the date of inspection, 01/08/2021, a SOR check from the State of Georgia was not available for review for Staff #3. The start date for Staff #3 was listed as 05/20/2020.
2. On the date of inspection, a SOR check from the State of West Virginia was not available for review for Staff #5. The start date for Staff #5 was listed as 09/08/2014.

Plan of Correction: All missing documentation shall be obtained and placed in the applicable staff file.
