The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.
- Comments:

A monitoring inspection was initiated and completed on 01/05/2020. The in charge person was contacted by telephone to initiate the inspection. There were 45 children present with 8 staff. The Inspector emailed the person in charge a list of items required to complete the inspection. The Inspector reviewed 3 children's records and 3 staff staff records submitted by the facility to ensure documentation was complete. Evidence:
Child #2 and #3s records do not contain documentation of immunizations on the first day of attendance. Child #2's first day of attendance was 8/14/2017and Child #3's first day of attendance10/07/2019.
Child #3's record does not include a physical by the 30th day of attendance.
