This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on January 13, 2021 and concluded on January 13, 2021. The director was contacted by telephone to initiate the inspection. There were no children present and one staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.
The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-665-520-B-11 Description: Based on review of children's records and interview, the center did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child #4, enrolled 12/16/2019, had documentation of a diagnosed food allergy dated 12/06/2020 from the child's physician.
2. The director acknowledged the diagnosed food allergy documented on the most recent child's physical form.Plan of Correction: Per the director: The child's allergy was listed as a dietary restriction originally. The recent doctor visit in December changed the dietary restriction to an allergy and I didn't catch it. I will request that the parents provide an allergy care plan for the record.
