Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on January 08, 2021 and concluded on January 08, 2021. The director or in-charge person was contacted by telephone to initiate the inspection. There were twenty-five children present and six staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and six staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #:	63.2(17)-1721.1-A
Description:	Based on record review and interview, the facility failed to ensure that every applicant for licensure as a child day center shall undergo a background check in accordance with the Code of Virginia subsection B prior to issuance of a license and every five years thereafter. Evidence: 1. The central registry check for Board Member #2 was last dated 08/07/2015. 2. The sworn statement for Board Member #2 was last dated 07/25/2015. 3. The administrator verified the dates of the central registry check and the sworn statement.
Plan of Correction:	I am going to include all board officers in my staff background check database. This database is reviewed monthly to determine any areas that need updating. The forms have been sent the Board Member and will be completed ASAP.
