Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced remote monitoring inspection was conducted on 1.15.2021. The director was contacted by telephone and a virtual inspection was conducted. There were 12 children present, ages six - twelve years of age, with two staff supervising. The Inspector reviewed two children's records, three staff records, the emergency drill log, meals and snacks, enhanced cleaning and sanitizing of toys and surfaces, and daily indoor and outdoor activities. The information gathered during the inspection determined non-compliance with standards that are documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of three staff records, not all staff had a repeat tuberculosis (TB) test/screening at least every two years from the date of the previous test/screening. Evidence: the record for staff #1 (date of hire: 5/14/2018) had a TB test/screening dated 4/16/2018.

Plan of Correction: Staff will get an updated TB test/screening.

Standard #: 22VAC40-185-70-A
Description: Based on observation, and staff interview, it was determined that not all staff had a file with all required documents. Evidence: 1) individual A was observed acting as staff, alone with a group of four children, and there was no record on file with required documents. 2) the records for staff #1 and staff #2 did not have written information to demonstrate that the individual possesses the education, staff development, certification, and experience required by the job position.

Plan of Correction: Will get the missing information and ensure all staff have a file with all required documents.
