This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on January 15, 2021 and concluded on January 22, 2021. The director was contacted by telephone to initiate the inspection. There were twelve children in the care of three staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and three staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word `corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 63.2(17)-1720.1-B-4 Description: Based on record review, program did not ensure that the results of a criminal history record information check and sex offender registry check from any state in which the applicant has resided in the preceding five years was obtained.
Evidence: 1. The date of employment for Staff #2 was 1/11/21. 2. The file did not contain sex offender check results from either of the two states in which she had resided in the previous five years. 3. The file did not contain criminal history check results from one of the states in which she had resided in the previous five years.Plan of Correction: Not available online.
