This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was conducted on 1/21/2021. The director was contacted by telephone to initiate the inspection. There were 56 children present and 11 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4pm on Friday 1/29/21. Please email me at morgan.bryson1@dss.virginia.gov with any questions.

Standard #: 22VAC40-185-160-A Description: Based on record review, the center did not ensure that each staff member submits documentation of a negative tuberculosis screening no later than 21 days after employment.
Evidence: Staff #1 (Date of Hire: 7/17/2020) had a tuberculosis test on file dated 8/21/2020.Plan of Correction: Going forward TB tests will be obtained within 21 days of employment.
Standard #: 22VAC40-191-60-B Description: Based on record review, the center did not ensure that all staff complete a Sworn Disclosure Statement prior to first day of employment.
Evidence: Staff #3 (Date of Hire: 12/11/2020) had a signed Sworn Disclosure Statement on file dated 12/14/2020.Plan of Correction: Going forward staff will sign sworn statements prior to their first date of employment.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the center did not ensure that Virginia Child Abuse and Neglect Central Registry search results were obtained within 30 days of employment for all staff.
Evidence: Staff #3 (Date of Hire: 12/11/20) does not have Virginia Child Abuse and Neglect Central Registry search results on file. The search request was sent on 1/9/2021.Plan of Correction: Going forward all new hires will have registry searches mailed out within 7 days of employment and will have results within 30 days of employment.
