Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on January 27, 2021 and concluded on January 27, 2021. There were six children present and two staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed two children?s records and five staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable licensing standards and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-191-60-B
Description: Based on a review of four records for staff, and an interview with administrative staff, the program failed to have all new staff complete a sworn statement or affirmation prior to employment. Evidence: Staff member 1 did not have a sworn statement in the staff record. Staff member 1 began work by January 7, 2021.

Plan of Correction: Administrative staff immediately had staff member 1 complete a sworn statement and affirmation. The statement was obtained for the record and shared with the licensing inspector.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of four records for staff, and an interview with administrative staff, the program failed to ensure that all new staff submitted to fingerprinting for a criminal history record obtained prior to the first date of employment. Evidence: There was only a Virginia State Police criminal history finding in the record of staff member 1 that was completed in 2018 for a prior employer. There was not any criminal history record finding from the Office of Background Investigation for employment which began by January 7, 2021.

Plan of Correction: Administrative staff immediately arranged for staff member 1 to make a fingerprint appointment at a fieldprint location. Verification of the appointment was sent to the licensing inspector. The criminal history record finding will be viewed and filed upon receipt. Also, it will be shared with the licensing inspector.
