Comments:
On 12/10/20 a virtual monitoring inspection was completed. Due to the COVID 19 pandemic the inspector was not on site. This inspection was completed via mobile video application as well as email and phone correspondence. During inspection there were 19 children present observed resting and playing freely supervised by 7 staff. The facility and emergency supplies and procedures were inspection. Staff and children's records were reviewed. Violations are indicated in the violation notice.

Description: Based on review it was determined that all required information is not maintained.

Evidence: During inspection Child A was missing: Health information.

Plan of Correction: I'll make sure I get it.

Standard #: 22VAC40-185-540-D
Description: Based on inspection of emergency supplies it was determined that required equipment was not maintained.

Evidence: During inspection a working battery operated radio was unable to be observed; batteries were needed

Plan of Correction: I have batteries and will fix it.
