Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 1/29/21 and concluded on 2/3/21. The owner was contacted by telephone to initiate the inspection. There were 79 children present and 17 staff. The inspector emailed the owner a list of items required to complete the inspection. The Inspector reviewed 4 children's records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and interview, the center did not ensure that staff who have lived outside of the state of Virginia within the past 5 years obtain a central registry check from that state.

Evidence:
1. Staff 1 (date of hire: 11/23/20) indicted on her sworn statement or affirmation that she has resided in the state of California within the past 5 years. The record for staff 1 does not contain the results of an out-of-state central registry search for the state of California.
2. The center owner confirmed that the record for staff 1 does not contain the results of an out-of-state central registry search for the state of California.

Plan of Correction: The center responded with the following: The out-of-state central registry search was completed for staff 1 upon hire; however, the results could not be located during the inspection. A new search request has been mailed and the results will be placed in staff 1's record.

Description: Based on record review and interview, the center did not ensure that a copy of the results of a criminal history record check and a sex offender registry check is obtained from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff 1 has a hire date of 11/23/20. Staff 1 indicated on her sworn statement or affirmation that she has resided in the state of California within the past five years. The results of a criminal history record check and a sex offender registry check were not available for staff 1 from the state of California.
2. The center owner confirmed that the required out-of-state background checks for staff 1 could not be located during the inspection.

Plan of Correction: The center responded with the following: The criminal record history search and the sex offender registry search were completed for staff 1 before hire; however, they could not be located during the inspection. Both required searches will be completed as soon as possible and the results will be placed in staff 1's record.
