Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Supervision and the importance of staff being able to identify where children in their care at all times, was discussed.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 01.25.21 and concluded on 01.29.21. The director was contacted by telephone to initiate the inspection. There were 89 children present and 23 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 6 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on observation, record review and interviews, the center did not ensure operational responsibilities of the license, as the center's activities were not maintained in compliance with the center's own policies and procedures that are required by these standards.
Evidence:

1. During the inspection three different staff members supervising 3 different groups of children did not give an accurate account for the children in the their care.
a. In the two year-old room the director asked staff how many children were present. The staff replied, "fourteen." The director confirmed there were fifteen children present.

b. In the preschool room, the director asked staff how many children were present. The staff replied, "thirteen." The director confirmed there were eleven children present.

c. In the school-age room, the director asked staff how many children were present. The staff replied, "fifteen." The director confirmed there were sixteen children present.


2. When asked for the center's "procedures for identifying where attending children are at all times," the director provided the center's face-to-name procedure." The procedure states, "Match the child's name with their face to verify the child is physically with you." The procedure gave instruction to do this upon arrival, when crossing thresholds and ongoing/throughout the day. The director confirmed that the listed accompanying form, titled, " Daily Child Tracking Sheet," is used to record when staff use the face-to-name procedure.

3. The director confirmed the face-to-name procedure is to be used so staff identify where children are at all times. Neither of the 3 staff referred to in #1, was observed using the face-to-name sheet when answering how many children were in their care.

Description: Based on observation the center did not ensure each restroom was equipped with the required supplies.
Evidence:
The bathroom in the Kiva did not have soap available.
