Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Consultation was also given on MAT medcation forms and authorization time frames.

Comments:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 1/29/2021 and concluded on 2/5/2021. The Director was contacted by email to initiate the inspection. There were 51 children with 16 direct care staff within ratios for supervision. Reviewed nutrition, a sample of children and staff records, trainings and emergency preparation. Medication was reported to be on-site the day of the inspection and reviewed authorizations and storage. FaceTime was used to view the infant classroom, nap time, bottle storage and diapering. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-510-A
Description: Based on medication authorization review, 3 of 3 authorization forms are incomplete as they are missing the parent signature.
Evidence:
Medication forms for Child #1 (2 forms) and #2 (1 form) do not include parent written permission/ signature to authorize the center to administer the medication.

Plan of Correction: The health and safety staff member will requrest the parent sign each form as needed.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on staff statement and record review, 6 staff do not documentation of a sex offender registry check from any state in which the applicant has resided in the preceding five years.
Evidence:
7 sex offender checks have not been obtained for 6 staff prior to 12/31/2020. (staff 1-6)

Plan of Correction: Registry checks were checked for all statt during the inspection and completed 2/5/2021.
