This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 2/8/21 and concluded on 2/9/21. The director was contacted by telephone to initiate the inspection. There were 52 children present and 15 staff present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 8 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 63.2(17)-1720.1-B-4 Description: Based on a review of eight staff records, it was determined that the provider did not ensure that the center obtain a copy of the results of a criminal history record information check and sex offender registry check from any state in which the applicant has resided in the preceding five years.
Evidence:
1. The record for staff #5 (date of hire 1/5/21) did not contain documentation of a completed criminal history record information check and sex offender registry check from the state of Illinois. The sworn statement or affirmation located in the record for staff #5 indicated that staff #5 had lived in the state of Illinois within the last 5 years.
2. The record for staff #8 (date of hire 2/1/21) did not contain documentation of a completed sex offender registry check from the state of New York. The sworn statement or affirmation located in the record for staff #8 indicated that staff #8 lived in the state of New York within the last 5 years
3. Staff #9 (Program Director) reviewed the records for staff #5 and staff #8, and confirmed that the results of a criminal history record information check and sex offender registry check from the states that each state had resided in within the last 5 years had not been received.Plan of Correction: The facility responded: We will reach out to Illinois and New York in regard to the criminal background check and Sex offender registry required for each staff. Going forward any new employee who has resided out of the state of Virginia within the last 5 years will complete all of the required background checks for that state prior to beginning employment.
