Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Please send your licensing inspector proof that the requests for the required background checks for Staff #1 have been submitted by Friday 2/12/2021. Documentation should include a copy of the form or request as well as proof of payment if applicable. Please ensure that you follow up with the appropriate agency if the results are not received back within 30 days following the request - and document these follow up calls, emails etc. in the record.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 2/5/2021 and concluded on 2/8/2021. The administrator was contacted by telephone to initiate the inspection. There were 28 children present and 5 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 63.2(17)-1720.1-B-4
Description: Based on a review of records and interview, the licensee did not obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state which the individual has resided in the preceding five years prior to their employment in the center.

Evidence:
- Three staff records were reviewed.
- The record for staff #1 who was hired 1/19/2021 contained a signed and completed sworn disclosure or affirmation form which indicated that staff #1 resided in Arizona and Washington within the previous 5 years.
- The record did not contain a criminal history record information check, a sex offender registry check, or a search of the child abuse and neglect registry or equivalent registry for Arizona or for Washington.
- When asked for this documentation, the administrator stated that she had not obtained them.
- The administrator confirmed that staff #1 was currently employed at the center at the time of inspection.

Plan of Correction: The Program Administrator stated that she will obtain Staff #1's criminal history record information check, sex offender registry check, and search of the child abuse and neglect registry or equivalent registry for the states of Arizona and Washington by Friday February 12, 2021.
