This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on January 29, 2021 and concluded on February 8, 2021. A complaint was received by the department regarding allegations in the areas of staff quantity and procedures for emergencies. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegations of non-compliance with standards or law and violations were issued. Any violations not related to the complaints but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-185-350-E-2 Complaint related: Yes Description: Based a review of documentation and interview, the center did not maintain the required staff-to-child ratio of one staff member for every five children age 16 months old to two years.
Evidence:
1. As indicated on the injury report dated 01/18/2021, staff #1 was alone in the "todds" classroom with eight children. Four of the eight children present were 16 months to two years of age (the youngest child present in the group was 16 months of age).
2. . As indicated on the injury report dated 01/21/2021, staff #1 and staff # 2 were in the "Twads" classroom with 14 children. Four children were 16 months to two years of age.
3. Administrator #1 stated that she was in and out of the classroom. Adminstrator #1 stated that she had to "step out to interview, that's when they are out of ratio". Standard #: 22VAC40-185-550-L Complaint related: Yes Description: Based on a review of documentation, the center did not ensure that notification was provided to the parent by the end of the day of any known minor injuries.
Evidence:
