63.2(17) License & Registration Procedures

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 2/1/2021 and concluded on 2/4/2021. The director person was contacted by telephone to initiate the inspection. There were 21 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of three children's records, the center failed to ensure that all physicals were obtained with in 30 days of enrollment.

Evidence:
1. Child # 2 started on 11/6/2020 did not have a physical examination within the 30 days of enrollment.

Plan of Correction: The director will have the parent to obtain a physical exam and will monitor all records.

Standard #: 22VAC40-185-60-A
Description: Based on review of three children's records, the center failed to ensure that all records were complete with all required information.

Evidence:
1. Children # 1 and 3 had their father as can emergency contact. An emergency contact is someone that can be reached when the parents can not be.

Plan of Correction: The director will have the parent to complete the missing items and no use the parent as a emergency contact.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of three staff's records, the center failed to ensure that all staff that has lived outside the state in the past five years had a criminal history record information check and sex offender registry check from any state in which the applicant has resided in the preceding five years.

Evidence:
1. Staff # 1 has lived out side of Virginia in the past five years. The staff person did not have documentation of the Child Protective Service check for Washington DC. Staff # 1 was hired on 7/13/2020.

Plan of Correction: The director will submit the out of state backgrounds ASAP. Will ensure that all new staff have all backgrounds completed.
