Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 2/4/2021 and concluded on 2/5/2021. The director was contacted by telephone to initiate the inspection. There were 32 children present and 8 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 5:00pm on Monday 2/15/21. Please email me at morgan.bryson1@dss.virginia.gov with any questions.

Violations:
Standard #: 22VAC40-185-550-E
Description: Based on record review and staff interview, the center did not maintain a record of the dates of all practice drills for one year.

Evidence: There was no documentation of an emergency evacuation drill in October 2020.

Plan of Correction: We will ensure that drills are documented each month.

Standard #: 22VAC40-191-60-B
Description: Based on record review and staff interview, the center did not ensure that all employees have completed a Sworn Disclosure Statement prior to employment.

Evidence: Staff #1 (Date of Hire: 12/11/2019) had a Sworn Disclosure Statement signed 12/30/2019 and Staff #2 (Date of Hire: 6/11/2020) had a Sworn Disclosure Statement signed 7/20/2020.

Plan of Correction: Moving forward we have double checked our staffing packets to ensure all needed documents are included at time of hiring and the orientation process.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, the center did not obtain the results of a fingerprint based national criminal history record search for all employees prior to employment.

Evidence: Staff #1 (Date of Hire: 12/11/2019) had fingerprinting results on file dated 12/12/2019.

Plan of Correction: Moving forward we will be certain to document the first date of employment based on receiving the required criminal history not the date the job was offered to employee.
