Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety inspection was initiated on 2/4/2021 and concluded on 2/5/2021. The director was contacted by telephone to initiate the inspection. There were 32 children present and 8 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility. Standard #:	22VAC40-665-540-A
Description:	Based on record review, the center did not ensure that the tuberculosis screening or test submitted by staff at the time of employment was completed within the last 30 days of the date of employment. Evidence: Staff #1: (Date of Hire: 12/11/2019) had a tuberculosis test on file dated 8/26/2019.
Plan of Correction:	Moving forward we ensure to have new hires meet the requirement for SHSI monitoring to have a TB screening or test completed within the last 30 days to be in compliance.
Standard #:	22VAC40-665-580-B
Description:	Based on record review and staff interview, the center did not ensure that all staff who work directly with children complete Virginia Preservice Training for Child Care Staff within the first 90 days of employment. Evidence: Staff #1 (Date of Hire: 12/11/2019) has not completed Virginia Preservice Training for Child Care Staff.
Plan of Correction:	Staff 1 will receive the training by 2/20/2021 and moving forward the office will ensure new staff receive the Virginia Preservice training for child care within the first 90 days of employment.
